marked "optional": incomplete forms will be rejected. When your form is accepted, you will be assigned a unique
Candidate Number. Please remember it!
Note: Submitting the form is simply
a declaration of interest on your part. It places no
obligation on you (to take the test) or on the examiners
(to let you write).
- Mail your fee promptly.
The fee for this examination is $100. On-line payment is not available, so advance planning is essential: the money
must arrive at the UBC Mathematics Department Office
by 15:30 on
Wednesday, June 8. Late payments will not be accepted; unpaid students will not be permitted to write the examination. There will be no exceptions.
